Health Services

Colorado Early Colleges health services are provided according to Colorado Department of Education guidelines. Health records are maintained by CEC and are kept in the student's cumulative file and/or health folder.

If your student is on a prescription medication, those medications are kept in the Health Office and will be dispensed, as ordered, by your health care provider (HCP). Please complete the Authorization to Administer Medication Form below. 

Please see below for more details and downloadable copies of all health forms requiring an HCP signature. All forms be must returned prior to the first day of school.

  • Authorization to Administer Medication - This form is to be signed by your HCP, giving the Health Office permission to administer any medication in school.  This form must be filled out and signed by a parent/guardian and your HCP if your child needs to take medication during school hours.
  • Colorado Allergy & Anaphylaxis Health Care Plan - When the student has an allergy requiring medication such as Benadryl or an EpiPen, this form is required to be completed by a parent/guardian and your HCP and returned to the CECFC Health Office before the first day of school.
  • Colorado School Asthma Care Plan - When the student has asthma and has a prescription to carry a medication (even if it is not all the time), these forms are required to be completed out by a parent/guardian and your HCP and returned to the CECFC Health Office before the first day of school.
  • Epilepsy/Seizure Action Plan - When the student has Epilepsy and/or Seizure Disorders, this form is required to be completed by a parent/guardian and your HCP and returned to the CECFC Health Office before the first day of school.
  • Diabetes - This is required to be filled out completely, by a parent/guardian and your HCP and returned to the CECFC Health Office before the first day of school.
Students are allowed to carry one day's dosage of over-the-counter medications. Medication MUST be in original packaging (bottle/container).  Students may not share ANY medication with other students.
